Test Form and Instructions

Problems??? Please try the following:

Get a FREE update to Adobe Reader v9.x or later.

Save Test Application to a file on your local disk and then use Adobe Reader to open.

Make sure in Adobe Reader that under Edit->Preferences that Enable Acrobat JavaScript is checked.

Notes

While the San Diego FSC hosts monthly test sessions and supplements our monthly sessions with test sessions at the Ray and Joan Kroc Community Center Ice Arena, many of our recent sessions have been very full. Please be aware of the following priorities:

USFSA Individual Members

Non Club Members

Test cancelations after the application deadline will forfeit the test fees.
All deadlines will be strictly enforced.
